When it comes to getting the right care at home, it can be difficult to know where to begin. We’re ready to help with the services and support that will help you live the life you want.

Contact My Aged Care

When considering your aged care options, you’ll first need to arrange an assessment through My Aged Care or call 1800 200 422.

They’ll chat with you about your day-to-day routine, physical and mental health, and what’s important to you.

You can also register a trusted person as a supporter who can assist and access information about your care.

Complete an Aged Care Assessment

My Aged Care will arrange an assessment for you. This will help determine which level of care is right for you.

You will receive a letter advising what care and services you have been approved to receive. You will then be added to the Support at Home queue.

Understanding the costs

Depending on your personal circumstances, Services Australia (Centrelink) will assess how much you will pay towards your services.

If you are already receiving a government payment, please call Services Australia on 1800 227 475. We recommend having your financial details ready before the call. If you have not contacted them before you will need to complete a financial assessment.

Accepting your Support at Home

When your funding becomes available, you will receive a letter notifying you that you have been allocated Support at Home funding.

You will have 56 calendar days from the approval date to choose an approved provider to support you at home.

Selecting your provider

When you choose Bolton Clarke, we’ll work with you (and your support person if you wish) to plan your expert care and support services and begin right away.

If you’re considering Home and Community Support, it’s important to understand the service and funding options available to you.

Home care services can be government funded, privately self-funded, or a combination of both. Your age, the level of care you require and your financial circumstances can help you determine which option will apply to you.

Funding options at glance

There are a number of state and federal government-funded programs designed to support your wellbeing and independence.

Each program will have specific eligibility criteria, and for some, access may depend on your location. We can provide step-by-step advice to help you navigate the application process.

Support at Home
program

This program provides government- funded home care across eight levels of support which can be used to cover services including: clinical supports; independence; and everyday living.

Commonwealth Home Support Program

CHSP is a government-subsidised program designed to assist those who are having trouble doing everyday activities and need low-care support. It also assists people to continue living independently.

Home And Community Care

In Victoria, the Home and Community Care Program for Young People (HACC PYP) provides basic support and maintenance services to help people under 65 who have disabilities live independently at home.

Private Funding

We can also provide private, self-funded services, either instead of – or while you’re waiting for – government funding. These can be arranged directly with Bolton Clarke and costs will depend on the services you request
